| Family Register Notification Forms |
|
Japan has a “koseki” family registration system that keeps a record of such information as marriages and childbirths. This registration system has no direct bearing on foreigners. However, notification forms must be filed for births and deaths that take place in Japan. Notification forms may also be required for marriages and divorces as well. If you marry a Japanese national, the marriage must be recorded in the family register system.
Please complete birth notification procedures at the municipal office of the birth or residence of the applicant within 14 days of the birth. You must then complete an application for foreign resident registration at the municipal office of the place of birth.
Please complete death notification procedures at the municipal office of the place of death or residence of the applicant within seven days from the day you noticed the death. In addition, please return the Certified Copy of Alien Registration of the deceased person to the Citizens Registration Section of Adachi City Office.
The requirements for marriage vary with each country. Please consult the embassy of your home country first.
The requirements for a divorce vary according to the country where the couple is living, as local laws may apply. Please consult the embassy of your home country first.
